This is nothing new. South Dakota folks living in smaller communities have had to journey to larger towns for medical care since before the state was organized. The USD Medical School was established to provide more Doctors and specialists for the state population at large. If the Doctors choose to locate in Sioux Falls, Rapid City, Aberdeen, etc. It is because of economy of scale and to build a large enough practice to earn a good living. This is just common sense. Not a shortage of Doctors.
